Hospital Patient Safety Report Card allows users to view performance of hospitals in Ontario in relation to patient safety, including hospital acquired infections and surgical safety. The hospital patient safety indicators are based on data collected by the Ministry of Health and Long-Term Care, which is reported to them by all hospitals in Ontario through the Self-Reporting Initiative (SRI).
