The April 2023 quarterly refresh of Home Health quality data has been updated on Care Compare Website, data updated April 26, 2023.

Quality of patient care measures:

Note: Claims-based measure calculations exclude Q1 and Q2 2020 data due to the COVID-19 reporting exceptions.

Managing daily activities (4 measures) – reporting data for 21Q3_22Q2

  • How often patients got better at walking or moving around
  • How often patients got better at getting in and out of bed
  • How often patients got better at bathing
  • How often a patient’s functional abilities were assessed at admission and discharge and functional goals were included in their care plan

Treating symptoms (2 measures) – reporting data for 21Q3_22Q2

  • How often patients’ breathing improved
  • How often patients’ wounds improved or healed after an operation

Preventing harm (6 measures) – reporting data for 21Q3_22Q2

  • How often the home health team taught patients (or their family caregivers) about their drugs
  • How often the home health team made sure that their patients have received a flu shot for the current flu season
  • How often patients got better at taking their drugs correctly by mouth
  • How often the home health team checked patients’ medications and got doctor’s orders for medication issues in a timely manner
  • How often the home health team began their patients’ care in a timely manner
  • How often patients experienced one or more falls with a major injury

Preventing unplanned hospital care (4 measures) – reporting data as follows:

  • How often home health patients had to be admitted to the hospital (20Q3_21Q2)
  • How often patients receiving home health care needed any urgent, unplanned care in the hospital emergency room – without being admitted to the hospital (20Q3_21Q2)
  • How often patients remained at home within 31 days of being discharged from home health (19Q3_19Q4, 20Q3_21Q2)
  • How often patients were re-admitted to the hospital for a potentially-preventable condition after discharge from home health (18Q3_19Q4, 20Q3_21Q2)
