Long Term Care Hospital Continuity Assessment Record and Evaluation Data Set

The Centers for Medicare and Medicaid Services (CMS) is preparing to make the Long Term Care Hospital Continuity Assessment Record and Evaluation (LTCH CARE) Data Set data available via the Research Data Assistance Center (ResDAC), alongside other CMS assessment data.  Initial release of data from Quarter 4 2012 through Quarter 4 2018. This is an all-payer dataset including, but not limited to, Medicare Fee For Service (FFS) and Medicare Advantage beneficiaries and includes risk scores and condition categories used to calculate risk scores for LTCH quality measure data.
