  1. Quality of patient care measures: Managing daily activities (3 measures) – reporting data for 17Q3_18Q2
  • Managing pain and treating symptoms (4 measures) – reporting data for 17Q3_18Q2
  • Preventing harm (8 measures) – reporting data for 17Q3_18Q2
  • Preventing unplanned hospital care (5 measures) – reporting data as follows:
    • How often home health patients had to be admitted to the hospital (17Q1_17Q4)
    • How often patients receiving home health care needed any urgent, unplanned care in the hospital emergency room – without being admitted to the hospital (17Q1_17Q4)
    • How often home health patients, who have had a recent hospital stay, had to be re-admitted to the hospital (15Q1_17Q4)
    • How often home health patients, who have had a recent hospital stay, received care in the hospital emergency room without being re-admitted to the hospital (15Q1_17Q4)
