Kaiser Health News (KHN) has published an online tool that allows users to check the scores (Medicare’s bonuses and penalities) for individual nursing facilities in their area. The federal government is issuing bonuses and penalties to skilled nursing facilities based on how often their patients are readmitted to hospitals within a month of being discharged. The KHN chart shows how much Medicare is increasing or dropping each payment over the fiscal year (ending September 2019). The changes do not affect patient whose stays are paid for by private Medicare plans, Medicaid or other forms of insurance.
