1. Quality of patient care measures (18 measures):
- Managing daily activities (3 measures) – reporting data for 17Q1_17Q4
- Managing pain and treating symptoms (3 measures) – reporting data for 17Q1_17Q4
- Preventing harm (8 measures) – reporting data for 17Q1_17Q4
- Preventing unplanned hospital care (4 measures) – reporting data for 17Q1_17Q4 except for 2 measures:
- How often home health patients, who have had a recent hospital stay, had to be re-admitted to the hospital (15Q1_17Q4)
- How often home health patients, who have had a recent hospital stay, received care in the hospital emergency room without being re-admitted to the hospital (15Q1_17Q4)
2. Patient survey results (5 measures) reporting data for 17Q2_18Q1

The quality of patient care star ratings are calculated by combining data for a number of individual quality measures, so they’re based on data from a combination of the data collection time periods shown above.
