Minnesota Department of Health (MDH) released its 12th annual report on Adverse Health Events in Minnesota. During the reporting time period of October 2014 to October 2015, there were 316 adverse health events reported to the Minnesota Department of Health (MDH). Twelve years ago, Minnesota established a statewide public reporting system for adverse health events (AHE). This law requires all hospitals, and ambulatory surgical centers, to report whenever an AHE occurs and to conduct a root cause analysis to identify the root causes of the event.
