The Pennsylvania Patient Safety Authority released its 2015 Annual Report – the results of a standardization project implemented April 1, 2015, involving multi-stakeholder workgroup to help develop a consensus of twenty-eight guiding principles to standardize event reporting. The guidance was developed to help provide consistent standards to acute healthcare facilities in Pennsylvania in determining whether occurrences within facilities meet the statutory definitions of Serious Events, Incidents, Infrastructure Failures and Other as defined in the Medical Care Availability and Reduction of Error (MCARE) Act.
