Wuesthoff Health System are asking residents of Brevard County who had a medical procedure performed from July 2007 through May 2008 to provide Explanation of Benefit (EOB) health insurance documents. They are gathering these EOB forms – with confidential information removed – for many types of procedures in numerous categories from 5 health plans. And the first person who submits an Explanation of Benefit (EOB) form that qualifies from each category receives a $100 gas card, to a maximum of three gas cards per person.
